These forms may be used free of charge.

Please be certain to download the correct form for both, the purpose and the issuing authority concerned. If you use the incorrect form for the purpose or issuing authority, the chances are that it will be regected.

	AARTO 04 Application to pay in installments	JMPD	THE REST
	Use this form if you want to apply for credit facilities to pay your fine but remember that in doing so the 50% discount gets taken away and you are applying to pay the 100% fine value off. Also remember that when the points-demerit system comes into play, points will be added to your licence as soon as you apply.
	AARTO 07 Nomination of driver or person in control	JMPD	THE REST
	Use this form if you were not driving the vehicle at the time that it was photographed or received a "parking ticket". Remember, you have 14 days to complete and submit this form. Owners of vehicles registered in the name of a juristic person (like a company or trust) that fail to complete and submit this form will pay 3 times the penalty value and may also get a hefty fine on top of it!
	AARTO 08 Representation	JMPD	THE REST
	Use this form if you had a valid reason for committing the infringement or if you have spotted deficiencies in the infringement notice itself. Whilst there are other reasons that you could be using this document, "pleading poverty" is not one of them. The AARTO Act makes provision for acceptance -in which case the notice is cancelled or rejection - in which case you remain liable for the infringement notice plus a R200 rejection fee if you complete this form after the 32 day discount period is up.
	AARTO 10 Notice of election to be tried in court	JMPD	THE REST
	Use this form if you want to be tried in a real court by a real magistrate instead of having some otherwise biased person whose qualifications and/or name you will never know "adjudicate" (judge) your matter.
	AARTO 14 Application for Revocation of Enforcement Order	JMPD	THE REST
	Use this form to have an Enforcement Order which never should have been issued in the first place revoked. The JMPD form may as well have never been authored since they have not, since the proclamation of AARTO in their jurisdiction, so much as issued a single one as at 20 December 2010.
	AARTO 25 Application for Refund of Monies	JMPD	THE REST
	Use this form if you have paid too much, paid the "wrong" traffic authority or are due a refund for any other reason.
